IVHSSPCA uses PayPal to process the Pet License form payment. There is a small fee when submitting online, which will be added to your license cost. For a one year license, this fee is less than $2.00. Required fields are denoted by an asterisk *. In order to complete your license application, IVHSSPCA requires a current rabies certificate and proof of alteration (if applying for the altered pet price). Applications will not processed without this information. Note: Veterinary receipts for vaccination will not be accepted as proof of vaccination and will delay the license application process. For the senior discount, proof of age is required. After completing the online form and submitting payment through PayPal please mail or fax the appropriate documentation to IVHS, Licensing Department.
